Grenth or Balthazzar?
Well yea basicley I can't choose between Grenth or Balthazzar forms. They both look cool but it's hard to choose
rend enchntments vs +40 armour
cold damage vs holy damage (which does more dmge?) and what does cold dmge actually do?

cold damage doesn't have any specific use except for the sake of being elemental damage. The only place I know for a fact that cold damage is desirable is hell's precipice.
Holy damage is harder to resist, and deals double damage to undead (mainly necro minions).
None of the avatars are "better" or "best". You go with what you need. Though typically grenth is more desirable in pvp, since it's not that often that you "need" enchantment removal in pve.

As far as general PvE goes, Balthazar is one of the better ones (though I use Dwayna and Melandru more) since you'll move way faster and take 1/2 damage from most sources (holy damage is also nice against warriors)
Grenth is nice but there aren't many situations where a enchantment will ruin your day as far as PvE is concerned

Grenth's is more for pvp, I wouldn't use it for pve. Balth's is okay, but you may as well run a warrior if the armours what you want. Or go earth ele. I'd go melandrus. Mels is good for wearying spam. Also good for chilling victory in pve.

Personally, I wouldn't waste my elite on either of them.
Dwayna/Lyssa are better for PvE, by... a long way.
If I had to say though... Grenth. It looks slightly cooler, and gets the awesome necro dance.

Matter of opinion here I think. Though I do agree that Grenth is kinda useless in PVE since removing enchantments and cold damage doesn't really help in many areas. I've used all that avatars quite a bit and they all have their uses. If a group lacks a warrior you can use Balthazar and be the tank. If you're in an area where the monsters constantly spam hex's you can use Dwayna, in a condition heavy area you can use Melandru etc etc... I personally use Avatar of Lyssa most often because of the extra damage to foes activating skills (which they do a lot in the later part of the game and in HM so you're almost always garanteed that extra damage bonus), and for the extra energy which also helps for spamming attack skills.

In the end, in PvE, once you put the SS aura on your bar, every derv is like christmas. 24/7 Dervish God Forms FTW! Yet, Grenth has little use compared to the others in PvE. Balthazar is more than likely to be a secondary tank, yet the other forms are better by circumstance. Preference, as Darkside said, will win the day in the end.

Balthazar is still useless. 33% move speed > holding agro. If your team needs a tank, use Armour of Earth or Conviction. Not wasting my elite slot on 16 more armour. And Holy damage is easily recreated using other skills.

Rending Touch and Rending Sweep. Not worth an elite slot when those 2 suffice. At least in PvE. Not like the rest of your team can't use enchantment removal either.

As for Balthazar because I haven't come across really annoying enchantments. Well spellbreaker maybe, but those mobs can be cut down fine anyway.

Avatar of Balthazar is +40 armor. So technically a Dervish using avatar of Balthazar would have more armor then a warrior...and combined with Eternal Aura you can keep the avatar up indefinitely. I don't see it as a waste of an elite slot, but as I said already matter of opinion if suppose.

My problem with balth is conviction.
Balth takes up an elite, has to be "cast", and you need 2 skillslots to keep it up 24/7.
Conviction is just 16 armor less, can be refreshed while using other skills and dealing damage, and can be kept up 24/7 with no effort at all. You can stack conviction with another avatar.
But yeah, just an opinion.

In my opinion, they're both garbage in PvE when compared to the other three.
Personally, I think that [skill]Reaper's Sweep[/skill] or [skill]Wounding Strike[/skill] are superior choices for 99% of PvE.

Cold damage for spinal shivers or something. Grenth pretty much owns face on a pain train but once the form goes down, like all others, you're useless.
In PvE I'd say go balth.
